Education Abroad Forms

For every USF student

To apply for education abroad programs at USFSP, you must submit the following items to the Education Abroad office in Bayboro Hall, room 204. Please contact the Education Abroad office at 873-4851 or send an e-mail to Wendy Baker, Interim Education Abroad Coordinator, if you have any questions or concerns.

  1. Study Abroad Application
  2. Participant Contract
  3. Medical Questionnaire
  4. Liability Waiver
  5. Course Approval Form
  6. Education Abroad Release Form
  7. Waiver of Reimbursement Statement
  8. CISI Insurance Enrollment Form

It is the students responsibility to get all of the signatures needed for the course approval form. A student must first see their academic advisor. He/ She will be able to let the student know how the course will fit with his/ her major. Once an advisor has singed it, the course approval should also be signed by the professor in charge of the trip.

To finish you application use our "E-shop" to pay your $300 or $500 deposit. Please print a receipt so that it can be submitted with your application package. This deposit is refundable only if you are not accepted on the program, or the program is cancelled by USFSP. To guarantee full consideration, submit your package before the due date. Applications will be accepted until the first day of classes.

Additionally:

  1. We will require that you demonstrate that you have insurance for illness or accidental injury valid outside the U.S. This type of insurance can be purchased though the Education Abroad office in Tampa. Click here to download the application.
  2. You may be asked to submit a brief (200 word) statement explaining why you would like to participate in the course.
  3. The name of a faculty member who we can contact as a reference.
  4. Semester Abroad programs will be run though the USF Tampa Education Abroad office and have additional requirements.

Non USF Students

Non USF Students are welcome to apply for our Study Abroad programs. In order to do so, one must first apply to USF as a non-degree seeking student. This will allow the university a way to track the student by giving him or her a "U number". For more information regarding this process please send an e-mail to studyabroad@stpete.usf.edu.

For Financial Aid

You may need to submit forms for Financial Aid. These forms will be available from USFSP Study Abroad office once you are accepted to the program. For more information go to our Funding Page.

Passports & Visas

    

U.S. citizens

If you do not already have a passport or if your passport will expire within 6 months of your expected foreign travel, you must apply for a passport. Passport Information & Applications

Check with Foreign Entry Requirements to verify if you will need a visa for your destination(s).

Non-citizens

Contact your Embassy in the U.S. to verify if a visa is required for your destination(s) abroad. Double check that you will be able to reenter the United States by talking to your foreign student advisor!
