How to Submit a Service Request (Work Order)
Service Request Priorities
The purpose of Service Request Priorities is to:
1. Provide a system to prioritize the use of limited staff and financial resources.
2. Provide equitable treatment to all clients.
3. Allow the orderly, timely and efficient accomplishment of facilities work.
When work is received it is assigned to one of three priority levels. Within each priority level work is assigned primarily on a first-in first-out basis, subject to efficient coordination of work and availability of material.
PRIORITY ONE
Hazards to Life, Health and Property
When work is required to provide or restore adequate security to the facility, to eliminate hazard to health and safety or to protect value of property, it is considered a Priority One. Examples would include a broken water line, widespread power failure, broken locks or perimeter doors, etc.
PRIORITY TWO
Essential Support
When work is needed to accomplish the essential business of the University’s mission of teaching, research and community service, to prevent a breakdown of essential operational or housekeeping functions, or to improve the operation performance of a necessary system, it is considered a Priority Two. This would include items such as lock failures in non-perimeter doors, partially clotted drains room temperatures that are too hot or too cold or tripped circuit breakers.
PRIORITY THREE
General Support
Routine work needed to accomplish the general business of the University is a Priority Three. This includes recurring maintenance and operations, routine maintenance request, items not of sufficient urgency to be considered Priority Two.
Keys and Locksmithing
Ordering Keys:
Key orders are initiated by sending a Key Authorization Form to Facilities Services. Key request are not taken over the phone or by E-mail. Please indicate the room number of each key required. Each individual requiring keys must fill out a key request form. Key request must be authorized and signed by department head/program coordinator.
PLEASE NOTE: Card key services, to include the authorization form, are not handled by Facilities Services. For more information on how to obtain a card key please contact Campus Computing by calling 873-4357.
