Undergraduate Academic Regulations Committee Petition Please follow the link to the ARC Petition form to begin the process. Make sure you schedule an appointment with you Academic Advisor regarding your petition.
Academic Regulations Committee Medical Form Please follow the link to the ARC Medical Form if your petition requires it.
Late Add Instructor Statement If you need to add a class late, please follow the link and download the form.

This is for the instructor to complete & sign.

Late Drop Instructor Statement If you need to drop a class past the withdrawal deadline, please follow the link and download the form.

Academic Renewal I If you were academically dismissed and are attempting to be re-admitted to USFSP, please download the Academic Renewal I form, if applicable.

You must meet with an Academic Advisor regarding the reinstatement process.
